Mechanical Removal of Calcifications on a Silicone Intraocular Lens with a Shielded Light Pipe in Asteroid Hyalosis

Authors: Naveen R Ambati, Ryan T Duong, Alice Zhang and Yevgeniy Shildkrot

Abstract: Calcific deposits on the posterior surface of a silicone intraocular lens (IOL) are a rare visually significant complication of asteroid hyalosis (AH). We report the novel use of a shielded light pipe to remove calcifications on a silicone lens. An 82-year-old woman with AH in the right eye and history of bilateral cataract extraction with Yttrium Aluminium Garnet (YAG) laser capsulotomies presented with visually significant calcifications on the silicone IOL of the right eye. Core vitrectomy was performed and a 27-gauge shielded light pipe with 0.045 mm between the tip of the shield and the light source was used to scrape calcifications off the posterior lens surface yielding a 5 mm central clearing.

The Impale Technique: An Approach for Softer Nuclei

Authors: Jeffrey Bloom, DO, Jacqueline Erdahl, DO, Caleb Liles, DO and Brian Mihok, DO, FAOCO, OFAAO

Abstract: We describe a novel cataract surgery technique that can be used for soft nuclei, which present difficulties for removal even for experienced surgeons. Such nuclei typically don’t crack as easily, tend to break into fragments, and don't allow for full thickness segmentation after grooving. After the nucleus is grooved with the phaco tip, subsequent steps are as follow

Minimally Invasive Transconjunctival Threading Method of Shortening the Müller Muscle and the Levator Aponeurosis in Severe Involutional Blepharoptosis

Authors: Kosuke Ogasawara, MD, PhD

Abstract: We have been performing involutional blepharoptosis repair by tucking the Müller muscle and levator aponeurosis with transconjunctival threading for over 10 years, while striving to improve this technique. The aim of the present paper is to introduce some progress in surgical quality.
